Over the last century, we added nearly thirty years to human life, but we never asked whether the brain would remain healthy enough to enjoy them.
Millions of people are living longer while experiencing prolonged periods of cognitive decline, emotional instability, disrupted sleep, and loss of independence. We have extended life, but not necessarily the brain's ability to support clear thinking, sound judgement, emotional balance, and adaptive behaviour throughout those additional years.
This disconnect exposes a fundamental limitation in how we measure successful ageing.
For decades, longevity science has focused on lifespan, the number of years a person lives, and, more recently, on healthspan, the years lived free from major physical disease. Missing from this framework is a measure of how long the brain remains capable of supporting the cognitive and emotional functions that make independent living possible.
Introducing Brainspan
A recently proposed concept known as brainspan offers a useful way to examine this challenge. Brainspan describes the period during which the brain retains sufficient functional integrity to support cognition, emotional regulation, sleep, adaptability and independent living.
Rather than presenting brainspan as a replacement for lifespan or healthspan, the concept invites us to ask an additional question: how long does the brain continue to support a meaningful and autonomous life?
Viewed through this lens, brainspan, healthspan and lifespan are deeply interconnected rather than arranged in a simple hierarchy.
Brainspan, Healthspan and Lifespan: An Interconnected Framework
When brain networks function well, individuals may be better able to maintain healthy behaviours, regulate emotions, manage chronic illness, make sound decisions and remain socially engaged. As brain function declines, the effects can ripple through many aspects of health, including medication adherence, financial judgement, relationships and physical independence.
In this way, brain health is one of the major influences on quality of life and on a person's ability to manage the conditions that affect longevity.
The Longevity Paradox
Modern medicine has succeeded in extending lifespan, but it has not guaranteed the preservation of brain function across those added years. As a result, many people spend an increasing portion of later life living with cognitive vulnerability.
This growing disconnect is what I call the Cognitive Longevity Gap.
The Cognitive Longevity Gap is the widening distance between how long people live and how long their brains retain the capacity for judgement, insight, learning, emotional regulation, and independent decision-making.
The gap often develops silently.
Changes in executive function, stress tolerance, emotional regulation, processing speed, and cognitive flexibility frequently emerge years before memory loss or a clinical diagnosis. Because these early changes are subtle, they are often dismissed as "normal ageing" rather than recognised as signs of declining neural resilience.
Why Midlife Matters
One of the greatest misconceptions about brain ageing is that it begins in old age.
Many of the biological processes that shape late-life cognitive health begin decades earlier. Midlife, roughly between ages 40 and 60, represents one of the most influential periods in determining long-term brain health.
Traditional thinking has portrayed brain ageing as a slow, linear decline beginning in our sixties. Neuroscience is revealing a far more dynamic story. Rather than deteriorating uniformly, the brain enters a period of heightened vulnerability during midlife, when the cumulative effects of lifestyle, metabolic health, chronic stress, sleep quality, and cardiovascular risk begin to influence the integrity of large-scale neural networks.
Structural changes accompany this transition. After approximately age 40, the brain gradually loses volume, averaging about 5% per decade, with the frontal and temporal lobes, regions responsible for executive function and memory, being particularly vulnerable.
Even more important are the functional changes. Emerging research suggests that large-scale brain networks responsible for communication, attention, and cognitive control begin to show increasing instability during the early forties. Rather than failing suddenly, these networks gradually lose efficiency long before symptoms become clinically apparent.
This period represents both heightened vulnerability and extraordinary opportunity.
Intervening at age 45 is fundamentally different from intervening at age 65. Later interventions often attempt to recover function that has already been lost. Earlier interventions help preserve neural function before significant deterioration occurs.
During midlife, the brain still possesses substantial neuroplasticity. Neural circuits can strengthen, reorganise, and adapt in response to physical activity, cognitive challenge, restorative sleep, stress reduction, social engagement, and metabolic health. These adaptations build cognitive reserve, the brain's ability to compensate for age-related changes and resist the clinical expression of neurodegenerative disease. For this reason, midlife represents a critical window for preventive intervention.
Rethinking Longevity
If we hope to improve the quality of ageing, we must move beyond treating disease after it appears and begin preserving brain function decades before symptoms emerge.
Protecting brainspan requires supporting the biological systems that sustain neural resilience:
- Regular physical activity
- Lifelong cognitive stimulation
- Restorative sleep
- Emotional regulation
- Meaningful social connection
- Metabolic health
- Effective stress management
Fortunately, the brain retains the ability to adapt throughout life. Through neuroplasticity, neural networks continually respond to our experiences and behaviours, making brain health one of the few aspects of ageing that remains modifiable well into older adulthood.
Longevity is no longer measured simply by the number of years we live.
The greater challenge is preserving the brain that allows those years to be lived with purpose, independence, and meaning.
Understanding Brainspan may be the key to closing the Cognitive Longevity Gap and redefining what successful ageing truly means.
The next frontier of longevity is not simply adding years to life. It is adding life to the brain that makes those years worth living.
